Мультиморбидность, нередко встречающаяся в педиатрической практике, сопровождается увеличением количества лекарственных назначений, поэтому необходим активный поиск инструментов, позволяющих снизить возможный риск и частоту нежелательных лекарственных взаимодействий у педиатрических пациентов.Цель работы: на примере клинического случая показать необходимость контроля в педиатрической практике совместного применения препаратов, в том числе с использованием лабораторного мониторинга фармакокинетических показателей.Материалы и методы: проведен ретроспективный анализ сведений из архивной медицинской документации ребенка 11 лет с нефротическим синдромом на фоне сопутствующего туберкулезного процесса, проходившего стационарное лечение с применением иммуносупрессантов в Российской детской клинической больнице в период с мая по июль 2018 г.Результаты: назначение циклоспорина для терапии нефротического синдрома послужило причиной проведения контроля концентрации препарата в плазме крови ввиду возможного фармакокинетического взаимодействия с лекарственными препаратами для лечения сопутствующего заболевания. Было выявлено взаимодействие циклоспорина и рифампицина на уровне биотрансформации. На фоне коррекции сопутствующей терапии (отмена рифампицина) отмечено достижение целевых значений концентрации циклоспорина в плазме крови ребенка, снижение протеинурии и гиперхолестеринемии, повышение уровня общего белка в крови, что свидетельствует об эффективности проводимой терапии основного заболевания.Выводы: полученные данные позволяют предположить, что лабораторное мониторирование фармакокинетических показателей в условиях полипрагмазии в педиатрической практике может повысить эффективность терапии, предотвратить нерациональное комбинирование препаратов и развитие нежелательных лекарственных взаимодействий при их применении.</p></abstract><trans-abstract xml:lang="en"><p>Nowadays, the problems caused by polypharmacy are recognised and widely discussed in the medical community. Multimorbidity, which is not uncommon in paediatric practice, comes with an increase in the number of prescriptions and necessitates an active search for tools to reduce the potential risk and frequency of adverse drug–drug interactions in paediatric patients.The aim of the study was to use a clinical case to illustrate the need for monitoring, including laboratory monitoring of pharmacokinetic parameters, during concomitant therapy in paediatric practice.Materials and methods: the study consisted in a retrospective analysis of the archived medical records of an 11-year-old child with nephrotic syndrome associated with a concomitant tuberculous process who had been receiving inpatient treatment with immunosuppressants at the Russian Children’s Clinical Hospital from May to July 2018.Results: the prescription of cyclosporine for nephrotic syndrome entailed monitoring of plasma drug levels for potential pharmacokinetic interactions with the medicinal products used to treat the concomitant disease. The monitoring revealed an interaction between cyclosporine and rifampicin at the level of biotransformation.
