Комбинированная психофармакотерапия у пациентов с коморбидной психической и соматической патологией создает высокий риск развития нежелательных реакций. Метод глобальных триггеров (Global Trigger Tool, GTT) позволяет проводить активный скрининг и количественную оценку нежелательных реакций, развивающихся под воздействием различных факторов в условиях рутинной клинической практики, и выявить направления для оптимизации терапевтической стратегии.</p></sec><sec><title>ЦЕЛЬ</title><p>ЦЕЛЬ. Оценить частоту и структуру нежелательных реакций при психофармакотерапии у пациентов психосоматического профиля, а также с помощью метода глобальных триггеров выявить ассоциации между применением отдельных препаратов, комбинированной психофармакотерапией и развитием нежелательных реакций.</p></sec><sec><title>МАТЕРИАЛЫ И МЕТОДЫ</title><p>МАТЕРИАЛЫ И МЕТОДЫ. Проведено одноцентровое ретроспективное исследование, в которое была включена 491 медицинская карта пациентов, госпитализированных в психосоматическое отделение в период с января 2024 по март 2025 г. Для выявления нежелательных реакций применен список из 27 триггеров, адаптированный для пациентов, получающих психофармакотерапию. Статистический анализ выполнен с использованием точного критерия Фишера.</p></sec><sec><title>РЕЗУЛЬТАТЫ</title><p>РЕЗУЛЬТАТЫ. Выявлено 25 типов нежелательных реакций. Наиболее часто у пациентов регистрировали кардиоваскулярные нарушения (n=211), антихолинергические эффекты (n=113) и гематологические реакции (n=89). Одновременное применение двух и более психотропных препаратов было ассоциировано с ростом частоты экстрапирамидных нарушений (до 14,1%, p&lt;0,001), острой задержки мочи (до 12,7%, p&lt;0,001) и артериальной гипотензии (до 9,4%, p&lt;0,001). Подтверждены известные ассоциации: антипсихотики — экстрапирамидные симптомы; антидепрессанты — кровотечения; противоэпилептические препараты — гематотоксичность. Сформированы новые гипотезы, требующие подтверждения: риск тромбоцитопении при приеме тригексифенидила (p=0,006) и перициазина (p=0,005); артериальная гипотензия при терапии вальпроевой кислотой (p=0,044); кардиотоксичность прамипексола (p=0,006) и леводопы (p=0,012).</p></sec><sec><title>ВЫВОДЫ</title><p>ВЫВОДЫ. Метод GTT эффективен для ретроспективного аудита безопасности психофармакотерапии. Комбинированная психофармакотерапия является независимым модифицируемым фактором риска. Выявлены новые ассоциации применения отдельных препаратов с развитием нежелательных реакций, требующие подтверждения и валидации в проспективных исследованиях.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>INTRODUCTION</title><p>INTRODUCTION. Combined psychopharmacotherapy in patients with comorbid psychiatric and somatic disorders creates a high risk of adverse drug reactions (ADRs). The Global Trigger Tool (GTT) enables active screening and quantitative assessment of ADRs developing under the influence of various factors in routine clinical practice and helps identify directions for optimizing therapeutic strategy.</p></sec><sec><title>AIM</title><p>AIM. To evaluate the frequency and structure of ADRs during psychopharmacotherapy in patients with a psychosomatic profile and to identify associations between the use of specific drugs, combined psychopharmacotherapy, and the development of ADRs using the Global Trigger Tool.</p></sec><sec><title>MATERIALS AND METHODS</title><p>MATERIALS AND METHODS. A single-center retrospective study was conducted that included 491 medical records of patients hospitalized in a psychosomatic department from January 2024 to March 2025. A list of 27 triggers, adapted for patients receiving psychopharmacotherapy, was used to identify ADRs. Statistical analysis was performed using Fisher exact test.</p></sec><sec><title>RESULTS</title><p>RESULTS. Twenty-five types of ADRs were identified. The most frequently recorded were cardiovascular disorders (n=211), anticholinergic effects (n=113), and hematological reactions (n=89). Concomitant use of 2 or more psychotropic drugs was associated with an increase in the incidence of extrapyramidal symptoms (up to 14.1%, p&lt;0.001), acute urinary retention (up to 12.7%, p&lt;0.001), and arterial hypotension (up to 9.4%, p&lt;0.001) that correlated with the number of psychotropic drugs used. Known associations were confirmed: antipsychotics with extrapyramidal symptoms; antidepressants with bleeding; and antiepileptics with hematotoxicity. New hypotheses requiring confirmation were generated: risk of thrombocytopenia with trihexyphenidyl (p=0.006) and periciazine (p=0.005); arterial hypotension with valproic acid (p=0.044); and cardiotoxicity of pramipexole (p=0.006) and levodopa (p=0.012).</p></sec><sec><title>CONCLUSIONS</title><p>CONCLUSIONS. The GTT is an effective method for retrospective safety audit of psychopharmacotherapy. Combined psychopharmacotherapy is an independent modifiable risk factor.
