Ламотриджин (ЛТД) — один из наиболее часто назначаемых противоэпилептических препаратов второго поколения. Препарат имеет низкий тератогенный потенциал, однако обладает генетически и метаболически детерминированными нейротоксическим, гепатотоксическим, дерматотоксическим эффектами и в некоторых случаях может вызвать полиорганную недостаточность. Понимание механизмов действия ЛТД с учетом фармакогеномики и фармакометаболомики, определяющих особенности его метаболизма, транспорта и элиминации у конкретного пациента позволит обеспечить индивидуализацию терапии и повысить ее безопасность.</p></sec><sec><title>ЦЕЛЬ</title><p>ЦЕЛЬ. Разработка подхода к терапии ламотриджином эпилепсии и других неврологических и психических заболеваний с учетом фармакогеномики и фармакометаболомики для снижения риска нейротоксичности препарата.</p></sec><sec><title>ОБСУЖДЕНИЕ</title><p>ОБСУЖДЕНИЕ. Метаболизм ЛТД осуществляется в печени глюкуронидацией (основной путь) и Р-окислением (второстепенный путь). В результате образуются как нейтральные, так и токсические (реактивные) метаболиты ЛТД, которые могут длительно циркулировать в крови, проникать через поврежденный гематоэнцефалический барьер у пациентов с терапевтически резистентными эпилептическими приступами и оказывать нейротоксический эффект, запуская или поддерживая механизмы нейродегенерации: нарушение нейротрансмиссии, синаптической пластичности, апоптоз нейронов. Большое значение в нейротоксичности ЛТД играют транспортные белки, участвующие в эффлюксе (выведении) токсических метаболитов из головного мозга в системный кровоток, а также из гепатоцитов в желудочно-кишечный тракт с желчью и через почки с мочой. Генетически детерминированное замедление эффлюкса препарата через гематоэнцефалический барьер (фармакогеномика) повышает нейротоксический потенциал ЛТД.</p></sec><sec><title>ВЫВОДЫ</title><p>ВЫВОДЫ. Для оценки риска ЛТД-индуцированных нежелательных реакций наряду с клинической оценкой состояния пациента целесообразно проводить: 1) терапевтический лекарственный мониторинг (кровь, волосы, слюна, грудное молоко); 2) анализ потенциально токсичных метаболитов (кровь, слюна, волосы); 3) фармакогенетическое тестирование носительства нефункциональных полиморфизмов генов, кодирующих ключевые белки-транспортеры и ферменты, участвующие в метаболизме ЛТД. Внедрение результатов фармакогенетического и фармакометаболического тестирования в клиническую практику эпилептолога позволит снизить риск нейротоксичности ЛТД.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>INTRODUCTION</title><p>INTRODUCTION. Lamotrigine (LTG) is among the most commonly prescribed second-generation antiepileptic drugs due to its low teratogenic risk. However, lamotrigine has pronounced neurotoxic, hepatotoxic, dermatotoxic potential (for genetic and metabolic causes) and in some cases can even cause multi-organ failure. Understanding lamotrigine mechanism can help individualise therapy and increase its safety, considering pharmacodynamics and pharmacometabolomics that determine its metabolism, transport, and elimination in a particular patient.</p></sec><sec><title>AIM</title><p>AIM. This study aimed to develop an approach to lamotrigine therapy of epilepsy and other neurological and psychiatric diseases reducing neurotoxicity, with due regard to pharmacogenomics and pharmacometabolomics.</p></sec><sec><title>RESULTS</title><p>RESULTS. LTG is metabolised in the liver in two pathways: glucuronidation (major pathway) and P-oxidation (minor pathway). As a result, neutral and toxic (reactive) lamotrigine metabolites are produced that can circulate in blood serum for a long time, penetrate the damaged blood-brain barrier in patients with therapy-resistant seizures and have a neurotoxic effect, triggering or maintaining neurotransmission disorders, impaired synaptic plasticity, neuronal apoptosis and other neurodegeneration mechanisms. An important role in lamotrigine neurotoxicity belongs to transport proteins involved in the efflux (excretion) of reactive (toxic) metabolites from the brain into the systemic circulation, as well as from hepatocytes into the gastrointestinal tract by bile and through the kidneys with urine. Genetically determined delayed efflux through the blood-brain barrier (pharmacogenomics) increases lamotrigine neurotoxic potential.</p></sec><sec><title>CONCLUSION</title><p>CONCLUSION. To assess the risk of lamotrigine-induced adverse reactions, together with clinically assessing patient’s condition, it is recommended to: 1) monitor drug distribution (blood, hair, saliva, breast milk); 2) analyse potentially toxic metabolites (blood, saliva, hair); 3) perform pharmacogenetic tests for non-functional polymorphisms of genes encoding key transport proteins and enzymes involved in drug metabolism. Является производным фенилтриазина (3,5-диамино-6-(2,3-дихлорфенил)-1,2,4-триазин) и применяется у взрослых и детей старше 12 лет в качестве моно- и/или политерапии фокальных и генерализованных эпилептических приступов, а также у детей старше 2 лет в качестве вспомогательной терапии при рефрактерном течении фокальной эпилепсии и синдрома Леннокса–Гасто [1–3]. ЛТД — первое ЛС с момента одобрения лития Управлением по контролю за качеством продуктов питания и лекарственных средств (Food and Drug Administration, FDA) для поддерживающего лечения биполярного расстройства I типа [<xref ref-type="bibr" rid="cit3">3</xref>][<xref ref-type="bibr" rid="cit4">4</xref>]. Есть данные, что ЛТД может иметь некоторую клиническую эффективность при нейропатической боли и головных болях1 [<xref ref-type="bibr" rid="cit5">5</xref>].</p><p>ЛТД быстро и полностью всасывается после перорального приема, незначительно метаболизируется в I фазе метаболизма в печени, а его абсолютная биодоступность достигает 98%. Максимальная концентрация ЛТД в плазме крови достигается в течение 1,4–4,8 ч после перорального приема и прямо пропорциональна введенной дозе в диапазоне 50–400 мг (терапевтический коридор в плазме крови варьирует от 4 до 10 мкг/мл) [<xref ref-type="bibr" rid="cit6">6</xref>]. Особенности фармакокинетики ЛТД и его низкий тератогенный потенциал способствуют повышению приверженности неврологов и психиатров к его широкому назначению, в том числе у женщин детородного возраста [<xref ref-type="bibr" rid="cit7">7</xref>][<xref ref-type="bibr" rid="cit8">8</xref>]. Однако концентрация ЛТД в плазме крови во время беременности снижается в среднем на 68% [<xref ref-type="bibr" rid="cit9">9</xref>], а клиренс увеличивается на 300% по сравнению с показателями до наступления беременности [<xref ref-type="bibr" rid="cit10">10</xref>]. При приеме беременными ЛТД в стандартных дозах эффективность терапии эпилепсии или биполярного расстройства снижается и возникает угроза жизни матери и плода [<xref ref-type="bibr" rid="cit11">11</xref>].</p><p>Недостаточные данные о фармакокинетике ЛТД во время беременности затрудняют корректировку его дозы [<xref ref-type="bibr" rid="cit12">12</xref>]. Показано, что площадь под кривой зависимости концентрации ЛТД от времени (AUC) существенно снижается во время беременности, поэтому рекомендуемая доза для I, II и III триместров беременности в 3, 3 и 5 раз соответственно больше базовой (принимавшейся до беременности). Однако для безопасности плода в I, II и III триместрах беременности максимальная рекомендуемая суточная доза при однократном приеме не должна превышать 400, 500 и 700 мг, а суточная доза при 2-кратном приеме — 300, 400 и 600 мг соответственно [<xref ref-type="bibr" rid="cit13">13</xref>].</p><p>Исследования последних лет свидетельствуют, что более высокие дозы ЛТД, поступающего через плаценту в кровь плода и с грудным молоком в кровь новорожденного/младенца, могут привести к серьезным негативным последствиям [<xref ref-type="bibr" rid="cit14">14</xref>][<xref ref-type="bibr" rid="cit15">15</xref>], включая гибель плода, тяжелую гипербилирубинемию с угнетением центральной нервной системы и даже асфиксию с гибелью новорожденного/младенца (при уровне в крови ЛТД ≥4,87 мг/л у ребенка первого года жизни) [<xref ref-type="bibr" rid="cit16">16</xref>].</p><p>Средний уровень ЛТД в пуповинной крови составляет около 60٪ от уровня в сыворотке крови матери при родах [<xref ref-type="bibr" rid="cit17">17</xref>]. Риск как для матери, получающей ЛТД, так и для новорожденного значительно повышается в послеродовом периоде, поскольку скорость выведения ЛТД возвращается к значениям, наблюдавшимся до беременности, в течение первых нескольких недель после родов [<xref ref-type="bibr" rid="cit7">7</xref>][<xref ref-type="bibr" rid="cit18">18</xref>]. По данным C.T. Clark и соавт. [<xref ref-type="bibr" rid="cit17">17</xref>], наиболее резко уровень ЛТД в крови возрастает в первые 1,5 недели после родов и далее варьирует в пределах от 30 до 640٪ по сравнению с его уровнем во II и III триместрах беременности. Средняя плазменная концентрация препарата в крови в первые 4 недели после родов на 154–402% выше, чем в III триместре беременности.</p><p>Фармакокинетические исследования уровня ЛТД в крови беременной женщины, плода, новорожденных и младенцев [<xref ref-type="bibr" rid="cit19">19</xref>], а также в грудном молоке [<xref ref-type="bibr" rid="cit20">20</xref>] важны для оценки токсичности и своевременной корректировки дозировки этого ЛС, но трудно выполнимы в реальной клинической практике. В данном обзоре представлены перспективные мультиомические методы (фармакогенетические и фармакометаболические) для прецизионного и безопасного назначения ЛТД [<xref ref-type="bibr" rid="cit21">21</xref>].</p><p>Цель работы — разработка подхода к терапии ламотриджином эпилепсии и других неврологических и психических заболеваний с учетом фармакогеномики и фармакометаболомики для снижения риска нейротоксичности препарата.</p><p>Поиск публикаций за период с марта 2010 по март 2025 г. (рис. 1) проведен в соответствии с руководством PRISMA 20202 в библиографических базах данных с использованием критериев включения/исключения, ключевых слов и их комбинаций на русском и английском языках (табл. 1). Всего проанализировано 134 публикации, из которых были исключены дублирующие публикации и публикации с отрицательными результатами. Всего в настоящий обзор включено 35 публикаций, отвечающих поставленной цели и критериям поиска.</p><fig id="fig-1"><caption><p>Рисунок подготовлен авторами по собственным данным / The figure was prepared by the authors using their own data</p><p>Рисунок 1. Not applicable</td></tr></tbody></table></table-wrap></sec><sec><title>ОСНОВНАЯ ЧАСТЬ</title></sec><sec><title>Белки-транспортеры</title><p>Имеются противоречивые данные о роли суперсемейства АТФ-связывающих кассетных транспортеров (ATP-binding cassette transporters, ABC) в эффлюксе (транспорте из головного мозга в кровь) ЛТД, в частности P-гликопротеина (P-gp), кодируемого геном ABCB1 (MDR1) [<xref ref-type="bibr" rid="cit6">6</xref>], и белка устойчивости рака молочной железы, кодируемого геном члена 2 подсемейства G ATP-связывающей кассеты (ATP-binding cassette sub-family G member 2, ABCG2; breast cancer resistance protein). Полиморфизмы генов, кодирующих этот белок-транспортер, могут оказывать существенное влияние на фармакокинетику и биодоступность ЛТД [<xref ref-type="bibr" rid="cit22">22</xref>].</p><p>В целом ЛТД идентифицирован как субстрат для двух основных белков-транспортеров эффлюкса через гематоэнцефалический барьер (ГЭБ) P-gp и ABCG2, имеющих синергическую или кооперативную роль в эффлюксе двойных субстратов [<xref ref-type="bibr" rid="cit23">23</xref>]. Эти белки-транспортеры могут играть важную роль в вариативной реакции центральной нервной системы на ЛТД, в частности у пациентов с эпилепсией, у которых оба транспортера сверхэкспрессируются. Кроме того, полиморфизмы в гене ABCC2 (MRP2), кодирующем белок канальцевого мультиспецифического переносчика органических анионов 2 (multidrug resistance-associated protein 2, MRP2), связаны с лекарственной устойчивостью в разных популяциях, что указывает на участие транспортера в эффлюксе ЛТД [<xref ref-type="bibr" rid="cit24">24</xref>]. Белок ABCC3 (MRP3) расположен на синусоидальной мембране гепатоцитов, отделяющих цитозоль от кровотока, и транспортирует глюкурониды ЛТД из клеток печени в кровоток для последующего выведения из организма почками [<xref ref-type="bibr" rid="cit25">25</xref>].</p><p>Механизм, посредством которого ЛТД пересекает ГЭБ, до конца не изучен, но выявлена роль транспортеров органических катионов (OКT) в переносе препарата [<xref ref-type="bibr" rid="cit26">26</xref>]. В частности, ЛТД является субстратом для белка-транспортера SLC22A1 (также известного как OКT1) [<xref ref-type="bibr" rid="cit27">27</xref>].</p></sec><sec><title>Рецепторы</title><p>ЛТД проявляет аффинность к нескольким типам рецепторов, экспрессирующихся в головном мозге. Он демонстрирует слабое ингибирующее действие на серотониновые рецепторы (5-HT2 и 5-HT3), слабо связывается с аденозиновыми рецепторами A1/A2 типа, адренергическими рецепторами (α1/α2/β), дофаминовыми рецепторами 1 и 2 типов (D1/D2), рецепторами к гамма-аминомасляной кислоте (ГАМК) A/B, гистаминовыми рецепторами (H1), κ-опиоидными рецепторами (KOR), мускариновыми ацетилхолиновыми рецепторами (mACh). Слабый ингибирующий эффект ЛТД имеет на сигма-опиоидные рецепторы3. Однако основная гипотеза, объясняющая противосудорожный эффект ЛТД, заключается в его связывании с потенциалзависимыми натриевыми каналами [<xref ref-type="bibr" rid="cit28">28</xref>], которые представляют собой гетеромерные комплексы, регулирующие обмен натрия между внутриклеточным и внеклеточным пространством в нейронах.</p><p>Считается, что ЛТД селективно ингибирует потенциал-зависимые натриевые каналы 2 типа (VGSC) [<xref ref-type="bibr" rid="cit29">29</xref>], стабилизируя мембрану нейронов, тем самым ингибируя пресинаптическое высвобождение возбуждающих нейромедиаторов (глутамата и аспартата). Альфа-субъединицы натриевых каналов в головном мозге человека преимущественно представлены четырьмя изоформами: SCN1A, SCN2A, SCN3A и SCN8A [<xref ref-type="bibr" rid="cit29">29</xref>][<xref ref-type="bibr" rid="cit30">30</xref>]. В состоянии покоя ионная пора натриевого канала находится в закрытом состоянии. Когда нейронные мембраны, содержащие VGSC, подвергаются достаточной деполяризации во время потенциала действия, происходит структурное изменение в белке, что приводит к открытию ионной поры. В течение нескольких миллисекунд после открытия ионной поры активируется петля инактивации, которая блокирует поток ионов через канал. В этот момент канал считается деактивированным. ЛТД связывается с VGSC в неактивированном состоянии, действуя как антагонист [<xref ref-type="bibr" rid="cit28">28</xref>].</p><p>ЛТД также может взаимодействовать с потенциал-зависимыми кальциевыми каналами (VGCC) N-, P- и Q-типов, кодируемых геном CACN, что определяет широкий спектр действия препарата. Каналы VGCC структурно похожи на VGSC, но они не имеют внутриклеточного цикла инактивации, характерного для VGSC. ЛТД ингибирует кальциевые токи Cav2.3 (R-типа), что может быть одним из его противоэпилептических механизмов [<xref ref-type="bibr" rid="cit31">31</xref>].</p><p>Хотя ГАМК-ергические рецепторы не считаются прямой целью ЛТД, они являются основными ингибирующими рецепторами в центральной нервной системе, а гетерогенность ионотропного рецептора ГАМК связана с эпилептогенезом [<xref ref-type="bibr" rid="cit32">32</xref>]. Глутамат является возбуждающим нейромедиатором, который играет важную роль в эпилептогенезе. Повышение уровня глутамата ассоциировано с инициацией судорог [<xref ref-type="bibr" rid="cit33">33</xref>]. ЛТД снижает уровень глутамата в головном мозге, ингибируя его чрезмерное возбуждение, с чем также связаны противоэпилептические и нормотимические эффекты препарата [<xref ref-type="bibr" rid="cit33">33</xref>][<xref ref-type="bibr" rid="cit34">34</xref>].</p></sec><sec><title>Фармакометаболомика и фармакогеномика</title><p>Основным путем метаболизма ЛТД является глюкуронидация, осуществляемая преимущественно ферментами системы уридилглюкуронидтрансферазы (UGT) UGT1A4 и UGT2B7 [<xref ref-type="bibr" rid="cit35">35</xref>][<xref ref-type="bibr" rid="cit36">36</xref>], принимают участие также UGT1A3, UGT2B15, UGT1A42, UGT2B72 [36–38]. Ферменты системы UGT играют важную роль в межиндивидуальной изменчивости метаболизма ЛТД.</p><p>В результате глюкуронидации ЛТД образуются следующие метаболиты: ЛТД-2-N-глюкуронид (основной метаболит, фармакологически неактивен), ЛТД-5-N-глюкуронид, ЛТД-2-N-метил и другие минорные соединения4 [<xref ref-type="bibr" rid="cit28">28</xref>][<xref ref-type="bibr" rid="cit39">39</xref>]. ЛТД-2-N-глюкуронид образуется в основном UGT1A4 [<xref ref-type="bibr" rid="cit12">12</xref>]. Известно, что вальпроевая кислота ингибирует метаболизм ЛТД в печени, увеличивая период его полувыведения линейно (с 24 до ~72 ч) с увеличением дозы [<xref ref-type="bibr" rid="cit28">28</xref>]. Увеличение отношения концентрации ЛТД в плазме крови к дозе (отношение К/Д) обусловлено конкурентным ингибированием ферментов UGT1A4 или UGT2B7 вальпроатами [<xref ref-type="bibr" rid="cit38">38</xref>] (вальпроевая кислота имеет более высокое сродство к UGT2B7 и также является субстратом для UGT1A4 [<xref ref-type="bibr" rid="cit39">39</xref>]), что важно учитывать при политерапии.</p><p>Два промоторных полиморфизма (rs3732218 −163G&gt;A и rs373221 −219C&gt;T) в гене UGT1A4 связаны со снижением активности фермента UGT1A4 на 40–50% и риском достижения токсических концентраций ЛТД в крови при приеме средних терапевтических доз препарата [<xref ref-type="bibr" rid="cit40">40</xref>]. Полиморфизм 142T&gt;G (rs2011425) в кодирующей области гена UGT1A4 ассоциирован с повышением концентрации ЛТД в крови и снижением его клиренса у пациентов с гомозиготным генотипом TT по сравнению с генотипами GT и GG [38, 41]. Полиморфизм rs2011425 (UGT1A4*3) гена UGT1A4 был связан со снижением отношения К/Д во время беременности [<xref ref-type="bibr" rid="cit42">42</xref>]. у пациентов с гомозиготным генотипом rs2011425 GG (распространенные или нормальные метаболизаторы) наблюдается более высокая активность глюкуронидации по сравнению с имеющими генотип TT (медленные метаболизаторы) [<xref ref-type="bibr" rid="cit43">43</xref>]. Полиморфизм rs6755571 (70C&gt;A) гена UGT1A4 связан с более высокой концентрацией ЛТД в крови и более низким клиренсом препарата даже во время беременности [<xref ref-type="bibr" rid="cit41">41</xref>][<xref ref-type="bibr" rid="cit42">42</xref>]. Редкий полиморфизм rs34946978 (1091C&gt;T) гена UGT1A4, который встречается у жителей Восточной Азии чаще, чем у европейцев и африканцев, связан со значительным снижением активности глюкуронидации ЛТД [<xref ref-type="bibr" rid="cit44">44</xref>]. Влияние различных гаплотипов на фармакокинетику ЛТД не изучено в полном объеме.</p><p>Минорный (редкий) гомозиготный генотип TT полиморфизма rs7668258 (−161C&gt;T) гена UGT2B7 ассоциирован с повышением отношения К/Д ЛТД по сравнению с пациентами с мажорным (распространенным) генотипом CC [<xref ref-type="bibr" rid="cit45">45</xref>]; у пациентов с генотипами TT и CT клиренс ЛТД в среднем на 18% ниже, чем у пациентов с генотипом CC [<xref ref-type="bibr" rid="cit46">46</xref>]. Исследование роли полиморфизма −372A&gt;G гена UGT2B7 показало, что клиренс ЛТД был на 247% выше у носителей мажорного гомозиготного генотипа GG по сравнению с носителями минорного генотипа AA [<xref ref-type="bibr" rid="cit47">47</xref>]. Описан клинический случай ассоциации полиморфизма −372A&gt;G гена UGT2B7 с дерматотоксичностью и полиорганной недостаточностью [<xref ref-type="bibr" rid="cit48">48</xref>]. Полиморфизм rs7439366 (802C&gt;T) гена UGT2B7 также связан со снижением отношения К/Д этого препарата у беременных женщин [<xref ref-type="bibr" rid="cit41">41</xref>].</p><p>У пациентов с мажорным гомозиготным носительством генотипа СС полиморфизма rs6755571 (70 C&gt;T) гена UGT1A42 (нормальные метаболизаторы) было отмечено снижение концентрации ЛТД на 22% по сравнению с носителями гетерозиготного генотипа СТ (промежуточные метаболизаторы) [<xref ref-type="bibr" rid="cit36">36</xref>]. Уровень ЛТД в сыворотке крови у пациентов с минорным гомозиготным генотипом TT rs7668258 (–161 С&gt;T) гена UGT2B72 (медленные метаболизаторы) был в 1,2 раза выше, чем у пациентов, гомозиготных по аллели C (нормальные метаболизаторы) [<xref ref-type="bibr" rid="cit36">36</xref>][<xref ref-type="bibr" rid="cit37">37</xref>].</p><p>Полиморфизм UGT2B15*2, который приводит к замене G&gt;T в гене UGT2B15 и вызывает замену аминокислоты с аспарагиновой кислоты (D) на тирозин (Y) в позиции 85 в ферменте UGT2B15, присутствует примерно у 50% населения европеоидной расы и в несколько меньшей степени у испаноязычных, афроамериканских, китайских, японских и корейских популяций (частоты аллелей в пределах 36–49%). Этот полиморфизм связан со значительным снижением глюкуронидации ЛТД и может быть клинически важным предиктором, определяющим межиндивидуальную вариабельность его клиренса. Показано, что у пациентов с гомозиготным генотипом ТТ сывороточный уровень ЛТД был выше на 18%, чем у пациентов с гомозиготным генотипом GG [<xref ref-type="bibr" rid="cit36">36</xref>]. Кроме того, одновременное применение других ЛС, ингибирующих UGT2B15, в частности вальпроевая кислота, может дополнительно повлиять на уровень в плазме крови и клиренс ЛТД и потенциально привести к токсичности ЛТД, особенно у беременных женщин во II и III триместрах беременности, новорожденных и детей первого года жизни [<xref ref-type="bibr" rid="cit7">7</xref>]. Роль полиморфизмов в гене UGT1A3, которые также могут влиять на глюкуронидацию ЛТД, пока недостаточно ясна [<xref ref-type="bibr" rid="cit37">37</xref>][<xref ref-type="bibr" rid="cit38">38</xref>]. Отчасти это связано с противоречивыми данными о роли кодируемого этим геном изофермента в образовании основного активного метаболита ЛТД-2-N-глюкуронида [<xref ref-type="bibr" rid="cit49">49</xref>].</p><p>В меньшей степени метаболизм ЛТД проходит при участии системы цитохрома Р450 печени. L. Liu и соавт. [<xref ref-type="bibr" rid="cit50">50</xref>] указывают, что изоферменты CYP2A6 и CYP2D6 ответственны за образование продуктов окисления ЛТД. При этом CYP2A6, по-видимому, является основным ферментом, активирующим ЛТД в микросомах печени человека [<xref ref-type="bibr" rid="cit51">51</xref>]. Дальнейшее образование активных метаболитов ЛТД происходит в кератиноцитах [<xref ref-type="bibr" rid="cit28">28</xref>]. Однако данные результаты неубедительны, поскольку не было найдено исследований, которые бы демонстрировали метаболизм ЛТД через систему цитохрома Р450 печени.</p><p>Несмотря на наличие в базе данных PubChem информации о метаболитах ЛТД, их биогенез и физиологическое значение остаются недостаточно изученными. Это указывает на необходимость проведения дополнительных исследований, направленных на изучение путей их образования и потенциальных функций в клеточных и молекулярных процессах.</p></sec><sec><title>Транспорт и выведение</title><p>Белки-транспортеры ABCB1 и ABCG2 играют ключевую роль в транспорте ЛТД через ГЭБ [<xref ref-type="bibr" rid="cit52">52</xref>]. Это позволяет предположить, что полиморфизмы в генах, кодирующих эти белки-транспортеры, ассоциированы с функциональными вариациями в активности эффлюкса ЛТД через ГЭБ и могут вносить вклад в межиндивидуальные различия в терапевтической резистентности и нейротоксичности ЛТД [<xref ref-type="bibr" rid="cit28">28</xref>].</p><p>Полиморфизмы rs1128503 (1236 C&gt;T), rs2032582 (2677 G&gt;T/A) rs1045642 (3435 C&gt;T) гена ABCB1 влияют на транспорт и концентрацию ЛТД в крови: носители гаплотипа 1236C-2677G-3435C имеют более высокие концентрации ЛТД в сыворотке крови по сравнению с носителями гаплотипа 1236T-2677G-3435T, за которыми следуют носители гаплотипа 1236T-2677T-3435C [<xref ref-type="bibr" rid="cit21">21</xref>].</p><p>Полиморфизмы rs2231142 и rs3114020 гена ABCG2 ассоциированы с сывороточной концентрацией ЛТД, нормализованной по массе тела [<xref ref-type="bibr" rid="cit21">21</xref>][<xref ref-type="bibr" rid="cit23">23</xref>]. Так, исследование роли полиморфизма rs2231142 (421C&gt;A) гена ABCG2 продемонстрировало, что гомозиготные носители минорного генотипа АА, получавшие монотерапию ЛТД, имели более низкую минимальную концентрацию ЛТД в крови, чем гомозиготные носители мажорного генотипа СС [<xref ref-type="bibr" rid="cit53">53</xref>]. Также показано, что этот полиморфизм ответственен за 4,8% вариабельности изменения сывороточной концентрации ЛТД у китайских пациентов с эпилепсией [<xref ref-type="bibr" rid="cit54">54</xref>].</p><p>Полиморфизм rs717620 (−24C&gt;T) в гене ABCC2 ассоциирован с резистентностью к ЛТД и риском нейротоксичности у европеоидов и азиатов, что, предположительно, является результатом компенсаторной активации транспортера ABCB1 [<xref ref-type="bibr" rid="cit55">55</xref>]. у китайских пациентов с эпилепсией, имеющих гаплотип ABCC2 −24C&gt;T / ABCC2 1249G&gt;A / ABCC2 3972C&gt;T, отмечен высокий риск развития резистентности к ЛТД [<xref ref-type="bibr" rid="cit55">55</xref>]. Однако полиморфизм −24C&gt;T гена ABCC2 не был связан с терапевтической резистентностью к ЛТД у пациентов с эпилепсией других рас и этнических групп (в частности, ханьцев, хорватов, австрийцев) [<xref ref-type="bibr" rid="cit28">28</xref>].</p><p>Показана ассоциация между концентрацией ЛТД в плазме крови и полиморфизмом rs628031 (1A&gt;G) в гене SLC22A1, кодирующем эффлюксный транспортер SLC22A1. Пациенты с минорным гомозиготным генотипом GG имели значительно более низкие концентрации ЛТД в плазме крови по сравнению с носителями мажорного гомозиготного генотипа АА, что указывает на связь этого полиморфизма с замедлением эффлюкса препарата из головного мозга в кровь [<xref ref-type="bibr" rid="cit54">54</xref>].</p><p>Белки ABCC3 и ABCC4 (известен также как MRP4) ответственны за транспорт метаболитов ЛТД из печени в кровяное русло5. Около 80% глюкуронидов ЛТД выводится с мочой (основной путь элиминации), включая ЛТД-2- (основной метаболит, 80–90%), ЛТД-N-глюкуронид, ЛТД, ЛТД [<xref ref-type="bibr" rid="cit50">50</xref>][<xref ref-type="bibr" rid="cit55">55</xref>] (табл. 2).</p><table-wrap id="table-2"><caption><p>Таблица 2. Однако минорная аллель А полиморфизма –1767G&gt;A в гене ABCC3 ассоциирована со значительным снижением экспрессии белка-транспортера ABCC3 в печени человека [<xref ref-type="bibr" rid="cit56">56</xref>], что может замедлять транспорт метаболитов ЛТД из гепатоцитов в кровь.</p><p>Транспортер ABCC4 (MRP4), кодируемый геном ABCC4, экспрессируется во многих органах и может изменять воздействие ЛТД или его метаболитов на клетки с различными последствиями. Локализуется в базолатеральной мембране эпителия сосудистого сплетения, а также на просветной стороне капилляров головного мозга [<xref ref-type="bibr" rid="cit57">57</xref>], замедляя поступление многих ксенобиотиков, включая ЛТД, в головной мозг, вытесняя их из кровеносных капилляров в плазму. Также ABCC4 локализуется на базолатеральной мембране гепатоцитов человека [<xref ref-type="bibr" rid="cit58">58</xref>]. Было проведено всего несколько комплексных исследований (в основном in vitro), посвященных влиянию экспрессии ABCC4 в печени на фармакокинетику ЛС, включая ЛТД [<xref ref-type="bibr" rid="cit59">59</xref>], поэтому его роль нуждается в дальнейшем изучении.</p><p>Согласно данным dbSNP6, для ABCC4 было зарегистрировано 30 185 полиморфизмов, две трети из которых расположены в интронных последовательностях. Некоторые из них имеют прямое или косвенное клиническое значение. Так, ABCC4 локализуется также на апикальной мембране проксимальных канальцев почек. Полиморфизмы rs1751034 и rs3742106 в гене ABCC4 играют важную роль в элиминации ЛТД с мочой, поскольку связаны с повышенным риском нарушения функции почек [<xref ref-type="bibr" rid="cit60">60</xref>], что может привести к более высокой концентрации этого ЛС в плазме крови и повышению риска развития нежелательных реакций (НР), включая нейротоксичность и гепатотоксичность.</p><p>Дополнительным путем элиминации глюкуронидов ЛТД является их выведение транспортером ABCC2 (также известен как MRP2) из гепатоцитов в желчь7. Информация о генах, кодирующих ключевые ферменты метаболизма и транспорта ЛТД, обобщена в таблицах 3, 4 и на рисунке 2.</p><table-wrap id="table-3"><caption><p>Таблица 3. Однако результаты некоторых исследований свидетельствуют о том, что повышение уровня глюкуронидов ЛТД в моче коррелирует с повышением уровня его активных метаболитов в плазме крови. Исследование глюкуронидов ЛТД в образцах мочи является неинвазивной и перспективной стратегией оценки безопасности терапии ЛТД, которая может быть реализована с использованием метода газожидкостной хроматографии с масс-спектрометрией или мицеллярной электрокинетической капиллярной хроматографии [<xref ref-type="bibr" rid="cit61">61</xref>].</p><p>Повышение в крови уровня активных метаболитов ЛТД, образующихся в результате Р-окисления (табл. 2), связано с развитием нейротоксических, дерматотоксических, гепатотоксических и кардиотоксических НР [<xref ref-type="bibr" rid="cit28">28</xref>]. Однако точный механизм их развития и степень токсичности этих метаболитов в зависимости от их уровня в крови, головном мозге (орган-мишень действия ЛТД) и других тканях требуют дальнейшего изучения.</p><p>Нейротоксичность ЛТД наблюдается редко [<xref ref-type="bibr" rid="cit62">62</xref>], проявляется в виде атаксии, головокружения и диплопии, особенно у новорожденных, детей первого года жизни [<xref ref-type="bibr" rid="cit63">63</xref>], женщин детородного возраста [<xref ref-type="bibr" rid="cit7">7</xref>] и пожилых пациентов [<xref ref-type="bibr" rid="cit64">64</xref>], но эта НР может привести к возникновению более серьезных проблем [65–68], включая эпилептические приступы (в том числе у людей без предшествующей эпилепсии), невропатию зрительного нерва с резким падением остроты зрения, угнетение сознания с оценкой по шкале комы Глазго ≤8 баллов, выраженную артериальную гипотонию, желудочковую тахикардию типа пируэт и остановку сердца (летальные исходы были зафиксированы при приеме 4 и 7,5 г ЛТД) [<xref ref-type="bibr" rid="cit68">68</xref>]. Нейротоксичность ЛТД зависит, кроме того, от генетических особенностей пациента, таких как полиморфизмы генов, кодирующих ключевые ферменты метаболизма и белки-транспортеры, участвующие в его выведении из головного мозга в кровь (фенотипы: медленные и промежуточные метаболизаторы и/или медленные и промежуточные транспортеры) [<xref ref-type="bibr" rid="cit66">66</xref>]. Связанные с этим межиндивидуальные генетически детерминированные различия уровней ЛТД в сыворотке крови и в головном мозге могут играть важную роль в развитии нейротоксических НР, поскольку замедление выведения ЛТД через ГЭБ может привести к накоплению токсичных метаболитов ЛТД в головном мозге при длительной терапии или при использовании высоких доз, а также у женщин детородного возраста в послеродовый период и их детей первого года жизни, находящихся на грудном вскармливании [<xref ref-type="bibr" rid="cit7">7</xref>].</p><p>В целом персонализированный мультиомический подход (фармакогеномика + фармакометаболомика) к применению ЛТД отражает современные тенденции клинической и лабораторной диагностики в неврологии и психиатрии [<xref ref-type="bibr" rid="cit66">66</xref>][<xref ref-type="bibr" rid="cit69">69</xref>]. Риск нейротоксичности у пациентов, которые являются медленными метаболизаторами и/или медленными транспортерами (гомозиготными носителями нефункциональных полиморфизмов генов изоферментов глюкуронидации и/или Р-окисления ЛТД (табл. 3) и/или генов, кодирующих ключевые белки-транспортеры ЛТД (табл. 4) соответственно) выше по сравнению с нормальными метаболизаторами и нормальными транспортерами (гомозиготными носителями полнофункциональных полиморфизмов генов, кодирующих ключевые изоферменты и транспортные белки ЛТД соответственно). Однако частота этих фенотипов среди пациентов, принимающих ЛТД, неизвестна, поскольку доступных исследований с использованием мультиомического подхода не найдено.</p></sec><sec><title>Оценка метаболизма и эффлюкса ламотриджина в клинической практике</title><p>Для оценки риска ЛТД-индуцированных НР наряду с клинической оценкой [<xref ref-type="bibr" rid="cit70">70</xref>] состояния пациента целесообразно проводить следующие мероприятия: 1) терапевтический лекарственный мониторинг (ТЛМ) ЛТД (кровь, волосы, слюна, грудное молоко) [<xref ref-type="bibr" rid="cit70">70</xref>]; 2) анализ активных (потенциально токсичных) метаболитов ЛТД в биообразцах (кровь, слюна, волосы) с использованием газожидкостной хроматографии; 3) фармакогенетическое тестирование (ФГТ) носительства нефункциональных полиморфизмов генов, кодирующих ключевые белки-транспортеры и ферменты, участвующие в метаболизме ЛТД [<xref ref-type="bibr" rid="cit66">66</xref>][<xref ref-type="bibr" rid="cit71">71</xref>].</p><p>ТЛМ позволяет обеспечить оптимальную корректировку дозы ЛТД. Перспективны для широкого внедрения в клиническую практику малоинвазивные стратегии взятия образцов для ТЛМ (высушенное пятно крови [<xref ref-type="bibr" rid="cit70">70</xref>] или плазмы [<xref ref-type="bibr" rid="cit72">72</xref>], слюна или волосы [<xref ref-type="bibr" rid="cit73">73</xref>][<xref ref-type="bibr" rid="cit74">74</xref>]), которые позволяют проводить забор образцов самостоятельно, хранить и транспортировать их при комнатной температуре. В некоторых клинических ситуациях (беременность, период новорожденности, старение, резистентность, токсичность) измерение уровня ЛТД и его активных метаболитов в плазме крови является достоверным и достаточным. В то же время для мониторинга безопасности и ранней диагностики токсичности препарата перспективна их оценка в крови, слюне и волосах методом жидкостной хроматографии с тандемной масс-спектрометрией. Исследование ЛТД в волосах также может быть использовано для оценки истории лекарственной терапии и определения долгосрочного соблюдения пациентом режима лечения. Концентрации ЛТД в слюне и плазме коррелируют (коэффициент корреляции r = 0,82, уровень значимости p &lt; 0,001); концентрация ЛТД в волосах коррелирует с концентрацией в плазме (r = 0,53, p &lt; 0,001) и суточной дозой препарата (r = 0,23, p = 0,024) [<xref ref-type="bibr" rid="cit74">74</xref>].</p><p>Оценить риск повышения уровня ЛТД и его активных метаболитов до токсического позволит проведение предиктивного (прореактивного) ФГТ для исключения гомозиготного (прежде всего) или гетерозиготного носительства нефункциональных полиморфизмов генов, ответственных за его метаболизм и транспорт [<xref ref-type="bibr" rid="cit66">66</xref>][<xref ref-type="bibr" rid="cit71">71</xref>][<xref ref-type="bibr" rid="cit75">75</xref>][<xref ref-type="bibr" rid="cit76">76</xref>]. Для медленных и промежуточных метаболизаторов и/или транспортеров рекомендуется отказ от назначения или снижение дозы ЛТД на 50 и 25% от средней суточной дозы соответственно, а также проведение ТЛМ в динамике 1 раз в 3 и 6 месяцев соответственно. При наличии технической возможности важно исследование уровня активных метаболитов ЛТД в биологических жидкостях (плазма или сыворотка крови, слюна, моча). При потенциально жизнеугрожающих симптомах токсичности рассматривается возможность применения методов экстракорпорального выведения ЛТД (гемодиализа, плазмофильтрации) [<xref ref-type="bibr" rid="cit77">77</xref>][<xref ref-type="bibr" rid="cit78">78</xref>].</p></sec><sec><title>ЗАКЛЮЧЕНИЕ</title><p>Проведенный анализ данных фармакогенетических и фармакометаболических исследований метаболизма, транспорта и элиминации ЛТД показал, что основными активными (потенциально токсичными) метаболитами являются ЛТД-N-оксид и ЛТД-N-метил, достижение высоких концентраций которых в крови и головном мозге (мишень действия препарата) может приводить к нейротоксичности. В группу высокого риска входят: новорожденные и дети первого года жизни, беременные женщины, пожилые люди, а также пациенты с неблагоприятным фармакогенетическим профилем (промежуточный метаболизатор и медленный метаболизатор), являющиеся гетерозиготными и гомозиготными носителями нефункциональных полиморфизмов генов, кодирующих ключевые ферменты глюкуронидации (UGT1A4, UGT2B7, UGT1A3) и, в меньшей мере, Р-окисления (CYP2D6, CYP2A6) ЛТД.</p><p>Снижение скорости выведения (эффлюкса) ЛТД из головного мозга в кровь у пациентов с неблагоприятным фармакогенетическим профилем промежуточный транспортер и медленный транспортер (гетерозиготные и гомозиготные носители нефункциональных полиморфизмов генов, кодирующих ключевые белки-транспортеры ЛТД: ABCC3, ABCC4, ABCC2, ABCB1, SLC22A1) также вносит существенный вклад в повышение риска развития тяжелых ЛТД-индуцированных нейротоксических НР.</p><p>Продемонстрированный подход с использованием фармакогенетических и фармакометаболических данных позволит более точно прогнозировать индивидуальные реакции на ЛТД и снизить риск развития токсических НР при его назначении. Прогресс в области омиксных технологий и использование искусственного интеллекта может облегчить внедрение мультиомического фенотипирования в реальную клиническую практику с учетом вклада фармакогеномики, фармакометаболомики, факторов окружающей среды, коморбидных заболеваний и образа жизни пациента в индивидуализацию психофармакотерапии.</p><p>Вклад авторов.
