Тирзепатид и семаглутид при сахарном диабете 2 типа и ожирении: сравнительная оценка эффективности и безопасности (обзор)

Резюме

ВВЕДЕНИЕ. В последние два десятилетия фармакотерапия ожирения и сахарного диабета 2 типа эволюционировала в сторону препаратов, воздействующих на инкретиновую систему, таких как семаглутид — агонист рецепторов глюкагоноподобного пептида 1 типа (ГПП-1) и тирзепатид — двойной агонист рецепторов глюкозозависимого инсулинотропного полипептида и ГПП-1. Оба препарата высокоэффективны в снижении массы тела и гликемическом контроле, однако различия в профилях безопасности определяют необходимость сравнительного анализа для персонализированного выбора терапии.

ЦЕЛЬ. Сравнительная характеристика эффективности и безопасности тирзепатида и семаглутида при сахарном диабете 2 типа и ожирении на основании данных клинических исследований и пострегистрационного фармаконадзора с выявлением ключевых различий в спектре и частоте нежелательных реакций для разработки стратегии пострегистрационного мониторинга безопасности.

ОБСУЖДЕНИЕ. В клинических исследованиях при применении тирзепатида отмечено более выраженное снижение уровня гликированного гемоглобина (разница составила от –0,15 до –0,44% в SURPASS-2) и массы тела (–14,7% в SURMOUNT-2 vs –9,6% в STEP-2) по сравнению с семаглутидом, данные реальной практики подтверждают эти результаты. По данным фармаконадзора (VigiBase, FAERS), желудочно-кишечные расстройства являются наиболее частыми нежелательными реакциями, характерными для данных препаратов (до 70%), риск тяжелых побочных эффектов, связанных с желудочно-кишечным трактом, сопоставим. Тирзепатид ассоциируется с более высоким риском рвоты, диареи и запора, для семаглутида выявлены специфические сигналы и нежелательные реакции: острое повреждение почек, неартериитная передняя ишемическая оптическая нейропатия, депрессивные расстройства и дизестезии.

ВЫВОДЫ. Тирзепатид и семаглутид эффективны для снижения уровня гликированного гемоглобина и массы тела (которые более выражены для тирзепатида). Профили безопасности препаратов в целом сопоставимы, но выявлены различия в спектре редких нежелательных реакций. Ввиду гетерогенности дизайна и ограниченного размера выборок результаты исследований влияния на щитовидную железу, костную ткань, а также фертильность требуют подтверждения в долгосрочных проспективных исследованиях. Необходим комплексный подход к мониторингу безопасности, сочетающий дифференцированный контроль рисков для тирзепатида и семаглутида.
